Enrollment Requirements

 

Documents required for enrollment include:

1.   An official copy of a student’s birth certificate is required for registration. 

Applications for an official birth certificate (VS - 37) can be obtained at the health department, principal’s office, or the central office. The cost is $10.00. The billfold size shall not be accepted. A certified copy (from Frankfort) must be presented to your principal; it shall have a seal pressed into the paper that verifies that it is a certified copy. Your school shall then make a copy to become part of your child's permanent record, and the official copy shall be returned. If the birth certificate is not available, please contact your principal immediately.

2.   Kentucky Immunization Certificate (EPID – 230a; 230b; 230c). All shots must be listed, the expiration date filled in and the certificate signed by a private physician or health department official. No photocopies will be accepted.

1.      Medical examinations must be on the KDE approved forms (KDE 1671 -410, dated 1999) and signed by a private physician or EPSDT Provider. Lab studies can be obtained at the health department. Head Start physicals are acceptable on

KDE approved form (KDE 1671 - 410) and signed by a physician or EPSDT Provider. Head Start/Preschool HCT/HGB, BP and US must be recorded.

4.   House Bill 706 requires that a vision examination be performed on all children enrolling in the first year of public school, public preschool or Head Start. The exam shall be performed by an optometrist or ophthalmologist and evidence given to the school of the exam by January 1 of the first year of enrollment.

5.   Legislation deleted requirements of parents and legal guardians to have children tested for tuberculosis (TB); required any child infected with TB only be examined and treated, instead of also tested, in accordance with Cabinet for Human Resources regulations; permitted a local health department, with the approval of the Department for Health Services, to require all first-time enrollees in a public or private school within the health department's jurisdiction to be tested for TB prior to entering school; deleted the requirement that each child entering public schools report TB skin test results to the local health department; exempted from TB testing any child whose parents object to such testing on religious grounds as so documented in a written statement, but in a suspected case of TB, a local health department may require testing of this child.*

 

Listed below are requirements for entry into Primary-1 (Kindergarten).

1.   Must be five (5) years of age by October 1st

2.   An official full-size copy of the student's birth certificate

3.   A valid Kentucky Immunization Certificate (EPID-230)

4.   A medical examination on the Kentucky Department of Education approved form

(KDE 1671-410 Initial Entry, dated 1999)

      5.   A vision examination performed by an optometrist or ophthalmologist

 

 

1. The medical examination must be on a KDE approved form dated January 1999 or later.

2. A current immunization certificate for Hepatitis B series must be submitted to the principal on or before the first day of school. 

3. All immunizations must be listed in order for the certificate to be valid.

 

 

A student shall be admitted to high school when that student has completed the prescribed elementary program of studies and has received a certificate of completion from a public, private, or parochial school. The administrator or teacher under whom the program was completed must sign this certificate.

Also, a Td (tetanus diphtheria) immunization is normally given at thirteen years of age and is needed at the high school level (due ten years after the last DTP immunization). All immunization dates must be listed in order for the certificate to be valid.

 

All transfer students seeking to enroll in the Floyd County Schools must provide the documents listed below to be enrolled.

1.   A Kentucky Immunization Certificate (EPID 230)

2.   A Medical Examination on a KDE approved form (KDE 1671-410)

3.   An official full-size birth certificate

4.   Proof of guardianship

5.   Proof of residence

6.   Official records from previous school

Pupils who transfer to Floyd County Schools from a home school or other non-accredited school will be assigned to the class or grade best suited for the student. This assignment may be based upon tests administered to the pupil by school personnel to determine grade placement or course credit. Previous credits earned by a student in a nonaccredited secondary school shall be awarded by the local school district by one (1) of the two (2) following methods:

a.       Pass an examination of similar nature and content to the examination used for other students receiving credit for a particular course within the school district and graded on a comparable basis; or

b.      Successful performance of the student in a higher level of the course when the courses are sequential in nature such as English, Mathematics, History and Science. Successful performance shall consist of achieving at least a C grade in the course by the 12th week of school.
